Here’s everything you’ll need to know for your child’s first appointment.

First Visit

During your child’s first visit, Dr. Witte asks that school-aged children be scheduled between 9:00 AM and 2:30 PM to allow enough time to review their dental health, X-rays, and any recommended treatment. Preschool-aged children are scheduled for the morning hours, when they are typically well-rested. If you need to cancel or reschedule, please call at least 24 hours in advance.

For a routine first appointment, you can expect:

Children 0–3 years old: A gentle exam of the teeth, gums, and bite with Dr. Witte and parent together

Children over 3½ years old: Cleaning, fluoride treatment, necessary X-rays, and a comprehensive evaluation by Dr. Witte.

After the exam, Dr. Witte will discuss his findings, any treatment recommendations, and his friendly front office staff will discuss scheduling and costs.

If treatment is needed. Families seeking emergency pediatric dental care may contact us any day—we strive to provide timely assistance.

For a routine first appointment, here’s what you can expect:
Children 1-3 years old will have their teeth, gums, and bite examined.

Children over the age of 3 and ½ will undergo cleaning, fluoride treatment, x-rays, and a thorough evaluation by Dr. Witte.

We’ll then discuss our findings and carefully go over any potential treatment plans and costs.

We work our hardest to make your child feel comfortable, so please allow about 45-90 minutes per appointment. Dentist appointments are an excused absence, so we will gladly write your child a pass for any missed school.

Payment Information

Most dental insurance plans will cover the majority of the cost of dental care, but there is a patient co-payment due for most services. Your child’s health is our priority, so we’ll discuss payment options with you before scheduling treatment. At John B. Witte, DDS, PC, we accept most major credit cards, CareCredit,  PPO insurance plans, and Medicaid / Chip plans. We will collect your estimated co-payment due at the time of your child’s appointment and if there is a remaining balance due upon receipt of payment from your insurance carrier we will send you a statement.  You are welcome to pay online, call our office to process payment by phone, or mail in your payment.

Patient Forms

To make your appointment as quick and easy as possible, please print and fill out the Parent Information, Acknowledgment of HIPAA, and Financial Policy forms prior to your visit and bring them to your appointment.

A separate New Patient History form is required for each child.

The Notice of Privacy Practices Form does not need to be brought to your appointment, but we have made it available to review for your convenience.
