New Patient Information

We know how stressful it can be to manage your child’s schedule, which is why our team works to make pediatric dental visits as convenient as possible. With our 30 + years of experience, John B. Witte, DDS, PC has learned the best methods for pediatric dental care to make sure your child is feeling their best during and after every visit. Here’s everything you’ll need to know for your first visit.

First Visit

During your child’s first visit to our office, Dr. Witte asks that school-aged children be scheduled between the hours of 9:00 AM and 2:30 PM to allow plenty of time for him to visit with you to discuss your child’s dental health and xrays, and treatment recommendations if needed. Preschool age children are scheduled during the morning hours when they are fresh and well-rested. We understand that things come up, so we ask that you please call our office at least 24 hours in advance if you are going to miss a scheduled appointment. For a routine first appointment, here’s what you can expect:

Children 0-3 years old will have their teeth, gums, and bite examined with Dr. Witte and parent together.

Children over the age of 3 and ½ will be scheduled for cleaning, fluoride treatment, necessary x-rays, and a thorough evaluation by Dr. Witte.

Dr. Witte will then discuss his findings and any potential treatment plans and costs with you after he has completed his evaluation. Our friendly staff will speak with you about scheduling and costs if Dr. Witte has recommended treatment for your child.

We work hard to make your child feel comfortable and have a pleasant dental visit, so please allow about 45-60 minutes for your first appointment. Dentist appointments are an excused absence, so we will gladly write your child a pass for any missed school.

This way, any children with homework or other commitments after school won’t have to worry about missing important work. Payment Information

Most dental insurance plans will cover the majority of the cost of dental care, but there is a patient co-payment due for most services. Your child’s health is our priority, so we’ll discuss payment options with you before scheduling treatment. At John B. Witte, DDS, PC, we accept most major credit cards, CareCredit,  PPO insurance plans, and Medicaid / Chip plans. We will collect your estimated co-payment due at the time of your child’s appointment and if there is a remaining balance due upon receipt of payment from your insurance carrier we will send you a statement.  You are welcome to pay online, call our office to process payment by phone, or mail in your payment.

Patient Forms

To make your appointment as quick and easy as possible, please print and fill out the Parent Information, Acknowledgment of HIPAA, and Financial Policy forms prior to your visit and bring them to your appointment.

The Notice of Privacy Practices Form does not need to be brought to your appointment, but we have made it available to review for your convenience.
